Such obvious deformities would only result in brother/sister, father/daughter or mother/son mating, or simply from very poor nutrition, or alcohol, drug or excessive tobacco use by the pregnant mother. The risk of such disabilities from cousins mating, even first cousins, is negligible. If cousins marrying is considered inbreeding, then all of us are inbred. If we checked our family tree going back several centuries, I'm sure we would find at least one union that involved close cousins. It was common all over the world, and still is in many cultures.

I've lived in the South most of my life, and cannot think of a single case where even distant cousins married. There is no evidence that this was more common in the Southern US. A lot of this is driven by anti-Southern prejudice in the media.
