Native Southern Italians/Islanders are better represented by the first photo, whilst native Northern Italians look more like those in the second photo.
However, I do think the first photo may be more representative in the modern day because of large-scale migration from South to North in Italy.
But if regarding native regional phenotypes, I say both are equal.

That looks like a pretty diverse mix of people from the different regions so I am going to say a little bit of both.

and phenotype wise, From those two sets, Set 1: southerns mainly S. Italy and Levantine
Second set:Central Europe mainly.

Obviously the majority would be somewhere in between both extremes, but the question is: which of the two extremes are more common?

The first one for the south of Italy mostly, for most of them being 'different' in terms of facial features compared to most other Europeans what enables me to have less trouble distinguishing them from let's say for example Iberians, French, mainland Greeks or south Slavs.
For the second set I'd have much more trouble doing that same thing^
I agree with Sikeliot by the way.
