Is the guy in the middle fully Italian?

Why? To begin with they are not just militants, they were young fascists killed in the late 70s in Rome during the so called years of lead.

They all seem to be of fully Italian ancestry, it's quite difficult to find better pics.

The guy in the middle was Francesco Ciavatta born in Montagano, Campobasso, Molise.

He doesn't look particularly Italian to me. I could be wrong of course. He has higher cheekbones than usual, the eye area.. maybe he's an extreme Alpine
