Those are almost Identical to a results of Half Sephardic-half Mizrahi guy I know

Here is the answer: Sicilian from Alcara li Fusi, Messina province.

I have NEVER seen anything like this but he has no traceable ancestry from outside that village. My guess is his ancestors consist of assimilated Jews.

Most Sicilians get 85-90% Europe, 10-15% MENA on 23andme.

Woah! I didn't expect West Asian to be that high even in the really south shifted Sicilian samples. The GEDmatch result would be very interesting.

Yes.

My friend is trying to contact this person to see if they can be uploaded to GEDMatch. Even I can admit they are an extreme outlier even though all Sicilians have Near Eastern input that is significant.

I expect them to be similar to this but even more eastern Mediterranean -- this is that friends' father.

Doesn’t seem possible that an ethnic Sicilian would get so little European.

There are 3 possibilities:

1. They are from a very isolated town in the middle of nowhere, so it is possible that there could be genetic or demographic peculiarities there, i.e. the towns in that area simply did not see a lot of migration from groups contributing to most of the European DNA in Sicily (Greeks, recent Italian input, Normans),

2. Their ancestors consist of assimilated Jews that they do not know about,

3. 23andme is not flawless at discerning between southeast Europeans and West Asians and is not a perfect system.

Thanks for the info.

The red dot is where they are from -- this might explain it to me because this area is not a huge population center, did not see as much migration, and may retain genetic peculiarities.
