Why Dinarids Get A Thicker Nose When Old?

When younger, his nose was very thin
but when older, his nose got much thicker:

I think this happens to a lot of people, regardless of their phenotype. Noses droop and get bigger as one ages. Ears get larger too.

But with that being said, this guy's case is rather extreme. His nose has gotten too bulbous and slightly deformed too. Did he have health problems?
