It's also most likely not natural masculinity. It looks like she has starved herself to achieve that. The abs are too visible to look healthy.

Ketosis is achieved by expending more carbohydrates than eating. This is a state the body goes into when the glycogen reserves in the liver are depleted.
When they are gone the body starts conserving its own glucose more, processing ketones for use as fuel in non-essential bodily expenditures.

Also, ketosis leads to less water accumulating in the muscles, hence easily losing more weight and looking less swollen.

Anyhow, for a genuinely muscular body, Ketosis is stagnation. Being muscular means being able to expend a lot of energy, and to do it fast, which requires access to glycogen stores.
For fat (triglycerides) to be used as energy the body will have to process them in order to make them usable.

The reason why an organism stores energy in the form of i. e. fat instead of having more glycogen reserves is because of the molecular properties of glucose in relation to water.

I don't know what you mean complete loss? Starving leads to more dramatic changes in fat cells than muscle cells (depending on body composition).

The muscles have atrophy when you have inadequate intake combined with low stimulation of muscle growth. She obviously has frequent stimulation by exercising.
