Actually, English are probably the majority, they just put down American. After that Scots-Irish, and then Germans.

The number of Americans with German ancestry is quite large, but many only have it in a small percentage. There are lot of Americans whose ancestors have been here for many generations who are of mostly British Isles descent (English foremost, with Scottish/Scots-Irish and a small bit of Welsh) but who also have considerable German ancestry from pre-Revolutionary War settlers—though English/British is almost always much bigger.
America, particularly the Midwest, Pacific Northwest, and some big cities, did however receive a lot of European immigrants throughout the 19th and early 20th centuries, and Germans were probably the biggest single ethnic group responsible. In my state, for example, German ancestry probably exceeds English because it was heavily populated by these "later" immigrants.
But my state would be an exception, and, all told, English has to be the biggest single ancestry in America.

No, it's a myth based upon an incorrect reading of census figures (due to misidentification of Anglo-Saxon Americans as "American" rather than "English"). There are at least 50 million Americans of English heritage (probably more).

It is impossible to go by surnames when deciding this because many people Anglicized theirs.
