I have a Prussian surname (certainly of Slavic origin) because my father is German. My ancestors were from Silesia (today a part of Poland). According to forebears, it's more prevalent in Germany and has the highest density in Germany (the eastern Länder, to be more precise). There's was a more accurate website but I can't find it anymore.
I have a quite famous homonym in Germany (we share the same surname AND first name, but as far as I know, we're not related); he's a screenwriter and author
2nd country: the USA (especially Pennsylvania, big bunch of German settlers there. Besides, I've heard of a colonel teaching at West Point who has the same surname, maybe a distant relative, who knows ?)
3rd: the Netherlands
4th: the Czech Republic
5th; Canada
Then Russia, South Africa, France (we're the only ones), Denmark and Austria

No surprise, according to Namespedia, my name is dominantly distributed in (East) Germany (149 persons registered, especially in Saxony and Thuringia, and also in Saxony-Anhalt)
2nd place: the USA (58 persons) this time Illinois appears to be the state where my surname has the largest distribution and the highest density, followed by Indiana, New York State and Washington State, then Idaho and Pennsylvania, Missouri etc...
The other countries where my surname is to be found are: Denmark, Austria, France (5 persons: me and my family), Switzerland, Norway (only one individual, actually, it's me again. I often work in Norway so I'm registered there too )
