    You're absolutely right - even WHO use questionnaire survey (alcohol consumption, drug consumption, etc.) and it is highly criticize because cultural differences play an important role - if people answer according to reality or lie (people from some cultures lie they drink/smoke marijuana/whatever more than reality is and some claims the opposite - they are ashamed of consumption). For Czech culture is typical that people exaggerate (every teen kid will tell you he/she smoke marijuana every day but they never even try it - they think it is something huge/big/they will be like bosses when they say something like that)....
    And that's the problem. As it goes for alcohol there is several litre difference per capita between "real" Czech consumption declared by sellers (taxes payers) and what WHO claims that the Czech consumption is...
