I would say Japan can't be No. 1. 3 years ago i calculated all-time most famous sports people (athletes, excluding managers) and Japan was in 11th place although Western countries could have been a slight advantage. In my classification UK is divided in 4 regions:

I've always been a list nerd. A while ago I considered calculating the most successful sporting nations, combining Olympic medals + World Championships in the most popular sports. For men, women, and combined.

You have to take into account population size.

Charles Murray's book on human achievement is a superior source.

EDIT: Or maybe they come to the same results. It would be interesting to check.

I've always been a list nerd. A while ago I considered calculating the most successful sporting nations, combining Olympic medals + World Championships in the most popular sports. For men, women, and combined.

I used wiki page popularity and added weights based on languages and decades when athlete was the most successful. I have another list for women - they are close to men by popularity only in some 5-6 sports.
I counted Olympic medals for the first time when i was 6 years old, and followed more from age of 10.
I made yearly list of countries by results in various world championships.
