Hypothetically, they should be willing to do whatever a consenting, informed patient wants and can afford.

But in reality, would they?

Like, for example, if a man who did not have gender identity issues wanted to have his perfectly healthy penis completely removed, would a plastic surgeon do that, assuming the prospective patient could afford it?

Or would the surgeon refuse, because most men have a subconscious but deeply rooted fear of emasculation, and the surgeon would feel uncomfortable doing such a thing, even if the prospective patient thought it would improve his body image?
