Italians and Iberians have a big phenotypical overlap, so the bulk of the white population in Latam wouldn't look drastically diferent if Italians colonized here instead of the Spaniards and Portuguese.
The only diference is that there would be more dinaric, alpine and East med genetics in Latam, but most white Latinos would look very similar to the current ones.
And unless Italy had unified it's territory in the XV/XVI centuries, I don't think Italian would be the main language south of Rio Grande, but one of the many regional languages of Italy: Sicilian, Lombardian, etc.
