A lot of those places i havent heard of here. Taco bell sounds good, i love nachos covered in stuff. But im gona pick from that list:

Subway - pulled chicken with letuce and tomato & bbq sauce, i can see them make it so i know they havent spat in it.

I avoid it myself but once a month or so I get some In-n-Out just to keep it real.
