I think people need to understand that the US is as much of a "Third World" country as anywhere else. It's hot and dusty in Iraq which gives it a certain look. It's also illegal to set up slums in the US, but that doesn't mean that people don't need them. Our marketing department is excellent at making it seem like the shiny bright things in the US are a symbol of wealth and health. Iraq has no marketing department.

First World your salary is good, but in compensation the cost of living is high (more expensive labor). The country or region is rich in money circulation, you will only live well, if have in region good quality public services and infrastructure.

First world is better for the poorest part of the population and for the upper middle class. Second world is better for lower middle class.

Yes, in practice it depends on what your salary is and what your expenses are. In the 2nd/3rd world in general, less job offers are expected, but this is relative depending on the unemployment rate and your profession (1st world in crisis has high rates too).
