Spanish in Spain and a mix of very basic Portuguese and Italian in Portugal (although I did use some English in Lisbon).

Everybody got that impression. I have discussed with some Italian user about it. Spanish tourists in nápoles speak the same again and again, it is very easy to check this in any web of travels.
We in Spain are used to see our streets, roads, buildings etc very clean, and it seems it is shocked for us places like nâpoles...

For work I go to Rome, Milan and Venice usually, and there I have not had issues with Italian. Growing up there were quite a few Sicilians and Calabrese where I lived, but I never had a chance to really pay attention to the way they spoke.
I think it's because the South has such poor management and corruption compared to the center and north.

Even if this were hard--that is how it is ! Assuredly, however, by far the harder fate is that which strikes the man who thinks he can overcome Nature, but in the last analysis only mocks her. Distress, misfortune, and diseases are her answer.

Spanish with italians and they italian with me=most of what we want to say is understood.
    In the north of Portugal=Galician.
In the south of Portugal=they don´t understand me in Galician, so Spanish.

That's odd, because Galician is even closer to Portuguese than Spanish is.

Well, it depends. In the past when visiting Portugal I just used my shitty "Portuńol", either that or English if they could speak it. I don't speak Italian so in Italy it was either speaking English or them speaking to me in Italian and me understanding about half.
