Because Recognition for this title was sought by the empire. Even the old Bulgarian word "tsar" (> Caesar, Caesar)/ king also had to be recognized by the empire, and this legitimized it before the whole medieval world. Any medieval ruler can claim such a title, but if it is not legitimized by the empire, it means nothing outside his own country.
