Meade you are closing the door on many a potential match with bias against their age and ethnicity. I have dated women of all ages and they are not the same just for being 20 or 40; people are individuals not numbers. My cousin had 2 children after the age of 50, after she married for years and no pregnancy occurred. You also have a fan of your music in Virginia, who is called Karen the Jewess lawyer. So consider to suspend your criteria for age and religion among others you have. If you would at least open the door to meet and become acquainted with a woman before passing judgment you may be very surprised.

Meade, you already have an established venue to meet women, which is your video programs. Have you considered inviting inviting guests to appear on your program? Guests, either appearing virtually or in person in your studio would allow you to interact socially and simultaneously increase the popularity of your programs. Begin at first with male guests and women you already know so to build confidence.

It seems it exists in the US too

Probably called a caregiver in the US. Someone who is paid by the state or a private company to help those who qualify and that help includes emotional support as well as physically caring/helping one who qualifies.

I know there are home health aides but I don't know they accompany their patients outside the house unless for doctor visits or food shopping. And it's usually for old people, not sure somebody with your condition can also have them.
