It's a skoptsy. Skoptsy were religious sect who castrated themselves to abstain from sin. I've noticed many skoptsy and castrati - though not all - look to have similar extremely elongated face like the acromegalics.

There are other curious similarities between acromegalics and castrated men. Both tend to grow very tall. Acromegalics of course don't produce testosterone either, and I don't know how the height is explained exactly in acromegalics. But the height of castrated men is explained so that when men are castrated in boyhood, that blocks injection of testosterone. Testosterone essentially ends childhood and stops body producing those other hormones which induce height growth. This means castrated men stay longer in that child like existence where you grow in height, and thus they grow taller.
